What type of visits are available?

Using your health insurance for nutritional counseling

Mary is a preferred provider for Medicare, Aetna, Cigna, United Healthcare, Midlands Choice.

Mary is a non-participating provider for Blue Cross Blue Shield.

Medicare only covers visits if you have Diabetes and/or kidney disease.

Medicare does not cover nutrition counseling for pre-diabetes.

If Mary is not a provider for your insurance

If Mary is not a provider for your insurance, payment of the cash price is due at time of visit by cash, check, or major credit card. Healthy Rise Nutrition will provide you with a Smart Bill for you to submit to your insurance for possible reimbursement to you.

Cash Payment Option Fees

Cash payment fees expected at time of booking visit. Pre-appointment forms expected at least 24 hours before visit.

Initial visit: $180

Follow up visits: $80 for 30 minutes

Healthy Rise will provide you with a Smart Bill for you to submit to your insurance.

Your insurance may cover the visit

Many health insurance policies cover nutrition counseling. This is often the case EVEN if you don’t have a diagnosis BUT want nutrition counseling for disease prevention.

Here’s how to check if your insurance covers nutrition counseling:
  • Call the 800 number on the back of your insurance card and speak to a representative.
  • Ask, “Does my insurance cover nutritional counseling?”
  • If the representative asks for a CPT code respond with the codes 97802 and 97803.
  • If the representative says you do not have coverage for those codes ask them to check coverage for codes 99401, 99402, 99403, and 99404.
  • If the representative asks for a diagnosis code tell them the visit is coded with the ICD 10 code Z71.3.
  • If Z71.3 is not accepted try the code Z72.4.
  • If you are overweight, obese, have pre-diabetes, diabetes, hypertension, or high cholesterol check on coverage for these diagnoses also.
  • Number of visits covered may range from 0 to unlimited depending on medical need and your insurance. Be sure to ask how many visits are covered.

Insurance Deductible

If you have an unmet deductible, we cannot bill your insurance directly. Payment is due at time of visit. You will be provided with appropriate documentation to submit to your insurance company to show the service provided. Once your deductible has been met, we can bill your insurance company directly.

Insurance co-pay

Most insurance companies consider nutrition counseling to be a specialist provided service. If this is true for your insurance, then the specialist copay is payable at the visit. The copay amount is often listed on your insurance card. Since we bill for preventative counseling, the copay might not apply. We can wait for the claim to be processed then charge your credit card with the copay amount.
